Learn more about expectations for Coordination of Care between Behavioral Health Providers (BHPs) and Primary Care Providers (PCPs), why it is important, and what BHPs may be able to do to improve Coordination of Care here. 

Integrated care is the best practice model that addresses the whole health of an individual. While full integration is not always possible, behavioral health providers (BHPs) have the opportunity to improve coordination of care across disciplines by collaborating with members and increasing outreach to primary care providers (PCPs). Through this collaboration, Magellan BHPs can improve the safety and efficacy of services to support better outcomes for members. Help members find medical providers who take Medicaid:

  • Visit VA Medicaid- Find a Health Care Provider.
  • Encourage or assist members to call the toll free phone number listed on the back their insurance cards.
  • Encourage or assist members to call Magellan at 1-800-424-4046 and request assistance with finding PCP.
